MAS West Midlands recently required the updating of their paper based Quality Assessment Questionnaire form to an online system.

I provided them with an online equivalent of their existing forms via one of the leading online survey system providers. But it didn’t end there, while the provider did offer a reasonable downloadable report document I took things a step further and created a template which strips the required data from the survey database and presents the key information in an easy to read and branded document ideal for providing quick overview.
